The mission of the Residential Services case manager is to provide short term, intensive case management to individuals in need of residential treatment services and successful discharge from services.



The service provides short-term intensive case management services, assuring the success of individuals seeking residential treatment. The case manager will manage the individual’s eligibility status for appropriate placement. Staff is in frequent contact with substance abuse detoxification and residential treatment facilities for booking bed space, conducting discharge planning, and meeting the client’s aftercare needs. The case manager also assures appropriate discharge planning and support for the individual successful return to the community.
