Vital Records, Birth and Death Certificates

Alexandria City Health Department can provide certified copies of death records for deaths occurring in Alexandria.  The cost is $12.00 per copy and an application for any vital record can be downloaded from the website.

Birth certificates: You may contact the vital statistics section of the Alexandria Health Department at (703) 746-4991, or contact the Office of Vital Records, P.O. Box 1000, Richmond, Virginia 23218-1000 at (804) 662-6200. The physical address for walk-in service is 2001 Maywill Street., Suite 101, Henrico, 23230. Applications are also available for out-of-state transit permits, disinterment and reinterment permits.
