Coordinated Care (Medicare and Cardinal Care Medicaid), Commonwealth Coordinated Care Plus

Commonwealth Coordinated Care Plus is a new initiative to coordinate care for individuals who are currently served by both Medicare and Cardinal Care Medicaid and can meet certain eligibility requirements. The program is designed to coordinate the delivery of primary, preventive, acute, behavioral, and long-term services and supports. In this program, the individual will receive person-centered care that is focused on their needs and preferences.

The goals are to improve quality care and health outcomes, streamline Medicare and Cardinal Care Medicaid requirements, reduce the burden for enrollees and providers, and more. Supplementary benefits will include care coordination, interdisciplinary care teams, and person-centered care plans.
