Who We Are
Honoring Choices Virginia offers residents of Virginia free virtual and in-person assistance with advance care planning, including the creation of new advance medical directives/living wills/healthcare POA forms and a review of existing documents.
They also offer free helpline assistance for individuals with questions or concerns related to advance care planning or patient capacity.
They provide free community education on advance care planning as well as medical orders such as Do Not Resuscitate (DNR) and Physician Orders for Scope of Treatment (POST) forms. The HCV website features many free digital and printable educational and planning resources for individuals.
