At Arlington County Community Services Board, Transition Team Services offers assessment and short-term, intensive case management, medical support and counseling services to adults who are homeless and have serious behavioral health disorders who are not yet linked to the long term Behavioral Health Care Division services. This service reaches out to individuals not yet connected to the system. The goal is to help individuals receive services in a timely way.
