Who We Are
The Child and Adolescent Case Management links children and their families to mental health resources, family supports, and community based services. Case Management coordinates services and assists with communication between service providers.
Services are flexible and can be provided at different levels of intensity from a once a month basis to a daily basis depending on child or adolescent's need.
Services include:
- Assessment of child or teen's needs,
- Referral to appropriate programs,
- Linkage to services,
- Supportive counseling,
- Problem solving,
- Resource development
- Service coordination,
- Education regarding resources, medication and emotional needs.
