At the Hampton-Newport News Community Services Board, Mental Health Case Management offers services for youths with behavioral health needs at home, in school, or in the community who need help in getting services. Case Managers help consumers and family members develop and carry out a plan to meet the needs, and assist them with school issues and the court system. The Case Manager links them to other services, assists them to get services, and arranges needed transportation. Case Managers assist youths who are being discharged from hospitals and residential facilities resume life in the community. Case Managers provide counseling to support daily living and resolve crisis.
