Sentara PACE, Norfolk

Sentara PACE, Program of All-Inclusive Care for the Elderly, is a comprehensive healthcare and support services program for frail seniors who wish to remain in their homes and community. It is an alternative to a nursing facility for eligible individuals who prefer to remain at home with services coordinated by Sentara PACE.



Sentara PACE provides care for participants, including comprehensive med...ical and rehabilitative services, in-home services and transportation. Sentara PACE is offered in Norfolk, Virginia Beach, Chesapeake, Suffolk and Portsmouth. There are two locations: Sentara PACE Norfolk and Sentara PACE Churchland.



The program is an approved Medicare and Medicaid provider, operated by Sentara PACE, a program of the Commonwealth of Virginia and the federal government.



You may be eligible to participate in the Sentara PACE Program if you are:

• Age 55 or older

• Able to live safely in the community

• Living in the Sentara PACE areas of Norfolk, Virginia Beach, Norfolk, Chesapeake, Portsmouth and Suffolk

• Qualify for nursing facility level of care by the Virginia Uniform Assessment Instrument (UAI)



Note: Participants may be fully and personally liable for the costs of unauthorized or out of PACE program agreement services.
