Skilled Nursing Care

Encompass Home Health professionals offer skilled nursing care to individuals in their home to maximize their recovery and quality of life. The staff works with each patient's physician to individualize a plan of care which can include specialized nursing, therapy to regain lost skills or assistance with daily living.

A wide range of skilled nursing services may be available in areas such as disease-specific management and education for conditions like heart failure, COPD, diabetes and orthopedic rehabilitation. We also assist in medication management and administration, and provide dietary and nutrition management.



Also available are private duty nursing services and social worker services. Encompass connect-Telehealth is offered if you are homebound and have frequent hospitalizations or trips to the emergency room.  Telehealth or remove vital monitoring uses a transmitter that connects to your phone wall jack to carry messages from wireless monitoring devices to a remote monitoring system. Examples are a blood pressure device, glucometer or PT/NR monitor.  Your vital signs are transmitted, information received and reported to your home healthcare provider.
