The Harrisonburg Community Health Center's Care Management Services provides assistance to clients living with HIV/AIDS in order to identify and facilitate access to and utilization of resources required to achieve and maintain the highest level of independence possible. Client services include:
- Individualized HIV Case Management
- Financial assistance in accessing HIV related medical services, medications, dental treatment, professional mental health services, and nutritional supplements
- Financial assistance with rent, mortgage and utility bills
- Assistance with transportation to medical appointments
- Access to onsite food pantry
- Condoms, dental dams, lubrication and pill boxes as available
