Who We Are
A program of Horizon Behavioral Health, Intermediate Care Facility (ICF-ID) services at Forest Hills provide intensive residential support at the highest community-based level. Services are similar to those provided in group homes with the addition of professional contractual staff such as Behavioral Psychologists, Dieticians, Education Specialists, etc. The facility offers nine beds.

ICF-MR services are provided for individuals at least 18 years old with a primary diagnosis of intellectual or developmental disability. Individuals must meet the ICF-MR/nursing home level of functioning and secure Medicaid ICF-MR funding.
