Who We Are
Halifax County Health Department offers pre-admission screening for long-term care facilities. For Cardinal Care Medicaid or Cardinal Care Medicaid-eligible individuals, screenings are done by a team consisting of a nurse and a social worker. The determination is made to see if the individual meets the criteria for nursing home placement, personal care services in the home or if some other community agency can best meet the individual’s needs. To have a screening, contact your local Social Services Department.
