Clarksville Primary Care is a satellite health center of VCU Community Memorial Hospital.  Staff provides out-patient care services and basic laboratory services to individuals from newborns to seniors.



Provides health care to the residents of the Clarksville area and offers geriatric assessment, cognitive assessment, ADL and IADL assessment, medication assistance, physical examination, nutritional and diet review, functional assessment, checking for sensory impairment, evaluation of falls, osteoporosis evaluation, evaluation and management of incontinence, screening for elder abuse, checking for hearing and vision impairment, depression, memory loss, anxiety, and immobility.  Nurse Practitioner or PA on staff.
