Who We Are
Provides assistance to individuals and their families to access services and supports that are essential to meeting their basic needs identified in the individualized service plan (ISP), which include not only accessing needed behavioral health services, but also any medical, nutritional, social, educational, vocational and employment, housing, economic assistance, transportation, leisure and recreational, legal, and advocacy services and supports that the individual needs to function in a community setting.
Provides intensive, moderate, and adjunctive case management services to adult citizens of Chesapeake, who are experiencing a mental illness in their natural environment (home, work place, shelters, etc).
The Chesapeake Assistance Trust enables families to provide resources for their disabled family member to ensure that their loved one's needs are met in the years ahead. Families are often concerned about the future of their loved one when they are no longer available to provide support and funds for their needs. This program provides an affordable way for families to provide for this.
