Case Management and Referral, Bedford

Johnson Health Center, Bedford Community Health Center, offers a case management and referral program as part of its mental health program.  You can use this service without being a current patient at Johnson Health Center.  The case managers can coordinate treatment plans for individuals with issues such as mental illness, eating disorders, or substance abuse.  The case managers assess the individual's mental states, help them set long term goals and connect them with the resources they need.
