Hospice of the Piedmont provides care wherever the patient call home--including private homes, nursing homes and assisted living facilities. Interdisciplinary teams of nurses, nursing assistants, social workers, chaplains, counselors and volunteers provide symptom and pain management for individuals living with a life-limiting illness.
Inpatient hospice care, during periods of medical crisis, can be provided in hospice-contracted beds in area hospitals and nursing homes, assisted living facilities as well as private homes.
Hospice provides palliative care approved by the patient's physician, along with emotional and spiritual support for the entire family. Working through teams made up of health care professionals, a coordinated program of care is planned to meet the unique needs of each patient and family.
Support from a nurse and a counselor is available to patients and families 24 hours a day. Grief counseling and support is available to all ages including "Journeys" - a program to aid children and teenagers cope with grief and loss.
