Who We Are
Capital Coordinated Medicine (CCM) can provide medical services to patients with multiple and complex health issues in private homes, independent living facilities, assisted living facilities and group homes. We offer access to blood draws, urinalysis and other points of care testing, many vaccines, tuberculosis screening and referrals for x-rays, EKGs.
The mission is to provide coordinated and advanced high quality medical care to frail and homebound patients with complex health problems.

To become a new patient, call the office or Click "Request an Appointment" on the website. Your insurance information and home bound status will be verified and "New patient paperwork" will be sent to you to complete and return. Call if you have questions.
