Southeastern Virginia Health System operates a Health Care for the Homeless program (HCH). This program was established to provide health care to homeless individuals in the Suffolk area. Case managers are available to assist with services that are free to eligible participants.
Medical care to homeless patients is provided by a physician, a physician's assistant, or a family nurse practitioner. If you or someone you know is homeless and in need of medical care and case management services, please call 757-380-8709 to schedule a screening.
