Who We Are
Hospice and Palliative Care (H&PC) services are provided which include nursing care, nurse's aide care, social worker assistance, as well as spiritual support for the patient and/or loved ones. Services include physician services for the medical direction of the patient's care, regular home care visits by Registered Nurses and LPNs to monitor the patient's condition and provide appropriate care and maintain patient comfort, and home health aids to provide personal care services. Hospice care is primarily delivered in the home, however, those living in adult care and nursing facilities may be served by H&PC of VA. The patient can be followed if hospitalization is required.
Hospice can be very beneficial to the patient and family due to the amount of teaching that is done to help loved ones assist the patient as their condition worsens. The hospice staff will guide you through this journey with step-by-step information. The goal of this care is to allow the end of the patient's life to be lived as they wish in peace and comfort. Spiritual support for the patient and their loved ones is also provided. Bereavement counseling for the family can be provided up to 13 months after the death of their loved one.
