Who We Are
A program of Horizon Behavioral Health, Program of Assertive Community Treatment (PACT) services are intensive community based mental health services provided to individuals who are in recovery from symptoms of a serious and persistent mental illness that require a combination of services in order to function optimally in the community. The combination of services that PACT provides includes psychiatric, medication management, case management, crisis, and community support services. Services are provided by a multi-disciplinary team of qualified mental health professionals. Services in PACT are provided 24 hours a day, seven days a week.

Intake Process
Call for a required appointment. Same day assessments for all ages are available Monday through Friday from 8:30AM-3:00PM. Same day assessments at the Bedford center are available on Mondays only.
