Who We Are
Carilion Clinic Home Care in Radford has Medical Social Workers or Case Managers available to assist patients and their families with the social, emotional and financial issues related to their healthcare needs. Social worker acts as liaison with medical staff regarding the plan of care, discharge planning, education, emotional support and counseling related to patient's condition.
Case managers may be involved with those with mental health needs, guardianship issues as well as those brought to the hospital due to circumstances resulting from trauma or crisis. May also provide counseling for the patient and the family to assist with solving problems which are preventing successful medical treatment.
